Do I Need a Prescription?
No need to worry we can take care of that for you! If your doctor hasn’t already provided an order, PMV can write a prescription for you on site. That way, you don’t have to delay or skip booking your appointment.
Most insurance plans typically cover:

4 bras every 3 months

1 silicone form every 24 months (per side)

1 non-silicone form every 6 months (per side)

1 post-op kit
Coverage varies by plan, but our team will guide you through every step so you get what you need without the stress.
Referrals & Orders
The best way to refer a patient to PMV is whatever works best for them. You may provide a written order for products, the order directly to us via pmvofal@gmail.com, or simply let us handle it we can provide a prescription on site. There’s no need to worry about paperwork, and this should never stop you from booking your appointment.

Eligibility: 2 forms for bilateral mastectomy, 1 form for unilateral mastectomy.
Important: All orders must include accompanying office notes that specify ongoing need (e.g., patient uses pocketed garments and prosthesis, patient has had a partial mastectomy, etc.).
